Premium Plus Membership β Terms and Conditions
This agreement governs the rights and responsibilities of clients enrolled in the Premium Plus Membership program (βMemberβ or βClientβ) offered by ErrandWorks.me (βCompanyβ). By enrolling in the Premium Plus Membership, the Client agrees to the following terms:
1. Monthly Wellness Check Entitlement
Each Member shall be entitled to one (1) wellness check per calendar month, which may be conducted either in person or via virtual consultation, subject to availability and mutual agreement. The purpose of said wellness check is to assess the Memberβs current needs and facilitate the coordination of applicable services.
2. Scheduling and Cancellation Policy
– All appointments must be scheduled in advance through the Companyβs designated booking system or by direct communication with authorized personnel.
– Members may cancel or reschedule a scheduled wellness check without penalty by providing a minimum of seventy-two (72) hoursβ advance notice.
– Rescheduling requests must be submitted via ErrandWorks.me/contact. In the event of a medical or safety emergency, Members are advised to contact the Company directly or dial 911.
3. Missed Appointment and Forfeiture Clause
– Failure to attend a scheduled wellness check without providing the required notice shall constitute a βMissed Appointment.β
– Missed Appointments may be rescheduled only within the same calendar month in which the original appointment was scheduled. Failure to reschedule within this period shall result in forfeiture of the monthly entitlement.
– Missed Appointments are non-refundable, non-transferable, and shall not roll over to subsequent months.
4. Courtesy Outreach
– In the event of a Missed Appointment, the Company may, at its sole discretion, initiate a courtesy telephone call to the Member for the purpose of wellness verification and notification of forfeiture status.
– Courtesy calls shall not be construed as a substitute for the monthly wellness check and shall not include service coordination or in-person support.
5. Discount Eligibility
– Members who miss a scheduled wellness check but remain in good standing may be eligible for discounted rates on future services, subject to availability and Company
